#####SPECIAL INSTRUCTIONS:
This digital notary will issue a certificate that ties your signature with
your publicly listed phone number and your name in such a listing. To qualify
for a certificate, you must have a phone number listed under your name that
can be verified by calling 411 or (area code)-555-1212 (from outside of your
area). In addition, when the listed number is called, there must be somebody
at the phone or an automatic announcement to confirm your signature ID.

IMPORTANT NOTE:
By submitting the notarization request to this digital notary and
subsequently accepting and using the digital certificate issued, YOU MAKE THE
FOLLOWING PLEDGE:
1) You fully represent the entity identified by the name and other
information in the notarization request and are duly authorized by the said
entity to execute the application for a digital notary certificate.
2) You acknowledge that the private key associated with the digital signature
submitted for notarization is fully under your control or the control of the
said entity or its authorized agent and that you and the said entity, not the
digital notary, is exclusively responsible for protecting the private key
from compromise, loss, disclosure, modification, or unauthorized use.
3) You and the said entity warrant that, when distributing any digital
content signed by the signature notarized by this digital notary, you and the
said entity shall exercise reasonable care consistent with prevailing
industry standards to exclude programs, extraneous code, viruses, or data
that may be reasonably expected to damage, misappropriate, or interfere with
the use of data, software, systems, or operations of the other party.

When submitting your signature, fill various fields in the notarization
request (the dialog box that will be shown when you click the "OK" button of
this dialog box) as described in the following:

1) Use the list box below the field labeled "Your Signature to be Notarized"
   to select the signature you want to be notarized.
2) In the box "Your name to appear in the certificate", fill the exact name
   listed in the phone listing.
3) In the box "Other information to be notarized", fill the 3 lines:
   phone: <your phone number>
   city:  <your city>
   state: <your state>
4) In the box "Information you want to submit to the notary but do not want
   to include in the notary certificate", fill the following information:
   a) The name and title of the person who is submitting the request.
   b) The name and title of the person who can confirm your signature ID over
      the phone or indicate that your signature ID will be announced by an
      answering machine or other automatic answering system (give directions
      on how to hear the announcement).
   You may add comments or other information in this box.
5) In the field labeled "How to send the certificate to you:", specify your
   Email address or regular(snail) mail address for the digital notary to
   send the certificate to you. You may specify both without extra cost.

When the notary receives your notarization request, it will first verify that
your phone is truly listed under your name by calling the phone directory
service. Then one of its examiners will call your phone number and ask the
person named in your request to confirm your signature ID or listen to the
announcement of your signature ID by an automatic answering system. When the
notary confirms your phone listing and your signature ID, it will issue a
certificate and send it to you by Email or regular mail according to your
request.

The fee for this notary is $25. The digital certificate is valid for 2 years.
The fee is fully refundable, if the notary fails to confirm the information
to be notarized and therefore cannot issue the digital certificate.

notary fee. To qualify for a certificate, you must pay the fee with a check
that has your name and other information you want to notarize.

To apply for a certificate, you must submit the notarization request in
electronic form (using "Submitting Notarization Request to Digital Notary"
dialog box of this program) and the following declaration in paper form
notarized by a Notary Public. The information entered into these two forms
must be consistent.
